TEAM 'OFFICIAL ROSTER' - REQUIRED
An approved, offical WYSA (or your state association) roster must be left with tournament officials at check-in and the tournament stamped roster at all games. Contact your club's registrar if you need to obtain such a roster.
PLAYER CARDS
All players must have a physical player card at all games if player photos are not on roster.

Which waiver forms are acceptable?
All teams MUST use the:
GUEST PLAYERS
Guest players are allowed at the same birth year or younger that they are guest playing for and are registered to teams in other clubs. Up to 3 guest players are permitted per team. Guest players must be listed on an approved roster from your state association: Wisconsin Guest Player Request
(Teams from other states attending WI tournaments should use their state's method of approving guest players for a tournament. Please refer to your state association website.)
CLUB PASSING PLAYERS
Same birth year players or younger registered to the same club as the tournament team can be 'club passing players', considered part of the team's roster. They are not counted as 'guest players' for that team. They will need to be on an approved roster which must be printed in additoin to the tournament team roster. **Club passing players must have player pass cards and waivers at the tournament.
You will upload into GOTSoccer all the following for Team Check in:
A) Official State Roster / or/ US Club Soccer Roster / or National organization roster (if other) : with all players that will be playing on the roster - with uniform numbers on the rosters [the rosters must be uploaded – we do not use the GOTSoccer generated rosters that some registrations systems use]- upload the official roster. NOTE: Guest players - can be written in on the rosters: all players need to have uniform numbers on the rosters. Guest players must have player card, medical release, and guest player form from your state association if required.
B) WYSA Medical Release forms for all players
D) Player ID cards- from a national/ state soccer governing body – such as USSSA, USYS State Associations, SAY Soccer, and US Club Soccer.
E) For teams outside of Wisconsin: Permission to travel per your national association policies if required. (note: US Club Soccer and USSSA rostered teams do not need permission to travel forms per their national association rules).
F) Teams will receive a stamped roster in return which they will need print 4 copies and bring to all matches. This will be emailed to the listed PRIMARY CONTACT on the GOTSoccer tournament team registration- if this needs to be a different person, please update your GOTSoccer application information.
Game Procedures: Teams will be responsible for having all rosters/ player cards/ medical releases. Team will provide a stamped tournament roster to the game officials of each game in addition to the player/ coach cards . Teams must be at the fields with all documents a minimum of 30 minutes prior to the matches.
